Serum Parathyroid Hormone and Vitamin D Levels as Predictors of Hypocalcemia after Total/ Near Total Thyroidectomy

Abstract Background: Post-operative Hypocalcaemia is the most-common complication of total and near-total thyroidectomy which a selective treatment for benign malignant thyroid diseases. Incidence ranges from 0.5-50%. Objectives: The role vitamin-D Parathyroid hormone(PTH) in incidence Hypocalcemia after has been taken into consideration. Methods: This prospective interventional study conducted Kasturba Medical College hospital, Manipal taking written informed consent participants. It aimed at surveying serum level preoperative Vitamin D, PTH calcium before total-thyroidectomy surgery its relationship with postoperative hypocalcemia surgery. was done on 70 patients who were-planned total/near thyroidectomy. Preoperative PTH, Postoperative 4 hours-PTH, Calcium were measured POD-1, POD-2-4, results obtained then analysed. Results: Considering cut-off as 8.6mg/dl, 42 developed 28 POD-2. levels people having hypocalcaemia where significantly less compared to normal calcium. 4-hours post-operative measurements showed 51% sensitivity, 100% specificity strong co-relation between drop levels- (p=<0.001). Out 28- (65%) had deficiency(p=0.5) POD-1 out 51 POD 2-4, 33(78%) had-vitamin-D-deficiency(p=0.3852). Which not statically significant. Conclusion:4 hours predictor early hypocalcemia, by detecting this we can effectively manage
